If you or someone you know is interested in becoming a GLS patron, please take the following steps.
Follow these four simple steps:
Eligibility
Read about eligibility to determine if the interested individual or institution is eligible for service.
Application
If eligible, complete the application form. Applications can be filled out by and or on the computer.
Signature by certifying authority
Have the application signed by a certifying authority. The certifying authority may be one of the following: Doctor of medicine, Doctor of Osteopathy, Ophthalmologist, Optometrist, Psychologist, Registered nurse, therapist, and professional staff of hospitals, institutions, libraries, and public or welfare agencies (such as an educator, a social worker, case worker, counselor, rehabilitation teacher, certified reading specialist, school psychologist, superintendent, or library staff).
Submit the Application
Submit application via email, mail, or fax.
